Registration and Insurance Coverage Verification
Your healthcare coverage must be verified prior to the initial appointment with your physician. The Registration Staff will contact you prior to your appointment to obtain all necessary information, to verify your insurance coverage and to obtain any pre-certification or authorization required by your health insurance.

Remember to bring your health insurance identification card to each appointment, as the Registration Staff will need to photocopy your identification card, and any other relevant information for our records.

If a specific treatment is recommended by your physician and you decide to receive the treatment at John B Amos Cancer Center, the required pre-certification or authorization must be obtained from your Insurance Coverage before the start of treatment. This may take several days to obtain for your specific treatment and will need to take place at the onset of treatment, and anytime your treatment plan changes in the future.

It is important that you understand the extent or limitations of your insurance coverage. If you provide all required information and we receive a written authorization for treatment (if required,) we will be glad to accept assignment and file your claim with your insurance company or healthcare organization for payment.

If your insurance does not cover your services you will be responsible for payment. You will also be responsible for any deductible and co-insurance determined to be your responsibility by your insurance coverage. Payment is due at the time of service for any amount determined to be the patient’s responsibility (deductible, co-insurance and non-covered charges).

Health Care Plans
The John B. Amos Cancer Center participates with Medicare, Medicaid, Tricare, Evergreen, Blue Cross Blue Shield and United Healthcare. (Note: The Radiation Oncologists do not participate with United Healthcare.) We also work with an extensive network of other insurance plans and managed care organizations. If you have questions regarding the various plans in which we participate (in-network or out-of-network), please call 706-320-8787.

Self Pay
If you are personally responsible for all or a portion of your charges, payment is expected at the time of service. If you are unable to pay your entire balance at each visit, please contact our Financial Advisor at 706-320-8829. The Financial Advisor will obtain your information and determine whether or not you qualify for one of our deferred payment programs.

Financial Assistance
The John B. Amos Cancer Center assigns a Care Coordinator to each patient, who will be your first point of contact to discuss financial assistance. If you have not been contacted by your Care Coordinator, please contact our office at 706-571-1687 and you will be put in contact with the Coordinator assigned to you. The Care Coordinators strive to meet all new patients but this may be impossible at times due to our volume.Your Care Coordinator will refer you to the Financial Advisor to assist you in applying for federal or state eligibility programs such as Medicare, Medicaid or Cancer State Aid. After review of your application and all required documentation if it is determined you do not meet the eligibility requirements for these programs, we have other financial assistance programs which may be of benefit to you. You must also submit an application and provide all required documentation to be eligible for any of these programs.

As the region's leading provider of healthcare, Columbus Regional Health has touched the lives of most of the children and adults in this region through its comprehensive network of services. Located in the West Georgia city of Columbus, Columbus Regional was established in 1986 as a not-for-profit health services organization, but its roots can be traced back to 1836 when the local hospital was a small wooden building on the banks of the Chattahoochee River.
