What to Bring

As soon as you’re referred to the John B. Amos Cancer Center, our staff will begin planning for your first appointment.

Here’s a checklist of items for you to bring:

  • Health Insurance Cards (including your outpatient prescription benefit card)
  • Photo ID (for example, a driver’s license)
  • Medications (bring a list or the bottles; include over-the-counter medications and any vitamin, mineral, herbal or natural supplement)
  • Questions you may have for Your Doctor
  • Documents requested by your Doctor

Located in the West Georgia city of Columbus, Columbus Regional was established in 1986 as a not-for-profit health services organization, but its roots can be traced back to 1836 when the local hospital was a small wooden building on the banks of the Chattahoochee River.
